** A late charge of 3.0% per month or 36.0% per annum will be assessed on all past due invoices $5 minimum
** This is not a contract; customer may cancel at any time by contacting our office
** We may electronically collect returned checks and charge fees as permitted by law
** All leased equipment must be returned upon termination of services.
** You the customer will be responsible for the cost of the equipment if it is not returned
** 3.5% additional charge on all credit/debit card transactions
** Minimum of 4 bottles is required for initial installment, no minimum is required for future deliveries.
** Residential Customers are required to pay for the 1st installment by credit card prior to set up.
** Residential Customers are required to participate in our "Auto Pay" program. We will charge the credit card at the 1st of each month for the balance due. An invoice and payment receipt will be mailed to the customer.
